- Your name & pronouns

- How you found me

- Where you would like to meet

- Number of hours

- Potential dates and times

- What kind of meeting you desire

- Any special wishes/details how you imagine our time together

- A photo of you

and

 - References from previous experiences are always greatly appreciated.
